Injury Log

This system is to be used only by Supervisors, Forepersons, or other appointed individuals specifically authorized (by the employer and EHS) for reporting and tracking employee injuries and illnesses.

 

Where should employees be sent for medical treatment of work related injuries?

Employees with work related injuries(or illnesses) are required to immediately contact their supervisor and advise them of their injury. If the injury requires medical treatment, the supervisor is to inform the employee of the University Preferred Provider for work related injuries and arrange for the employee to have the injury evaluated. Currently the university has an arrangement with OSF to provide evaluation and treatment for work related injuries at the OSF Occupational Health Center, Prompt Care and the OSF St. Joseph Medical Center Emergency Room. Employees are encouraged to seek treatment through OSF when injuries are of a nature that requires medical care.

The injured employee must bring with them an "Authorization for Medical Treatment" form completed and signed by their supervisor or other appointed individual. This form includes directions on how to get to the OSF facilities.

If you are not an employee and would like to submit an accident report, please complete the "Non-Employee Accident Report" and submit the form via campus mail or USPS to:

Campus Box 1320, Normal IL 61790-1320.

Forms can also be picked up on campus at Environmental Health and Safety- 202 Nelson Smith Building (located directly west of Redbird Arena).
